[*] Scope of Service:

(1) Vendor needs to provide temporary nursing personnel service to the government authority located in Rochester, NY.    
- Retain all necessary professional licenses or registrations for its employees during the term of the resulting Agreement. Upon request, the selected Respondent(s) will provide agency with copies of these documents, as well as verification of health exam, including Measles, Mumps and Rubella Vaccination or Titer, T‐spot or Purified Protein Derivative (PPD) results (x2), Influenza vaccination, and the Hepatitis B declination along with Hepatitis B vaccination or titer. Although covid vaccination is not currently required, vaccination status is required.
- Ensure that all work performed is in accordance with agency regulations.
- Ensure and be responsible for the mandatory educational needs (12 hours each year) for the recertification of all agency.
- Ensure that all agency and agency pass a medication exam that has been mutually agreed upon by agency and the selected Respondent(s). Ensure all agency contract staff members maintain valid Basic Life Support agency certification   
(2) All questions must be submitted no later than September 29, 2023.
(3) The contract will be one year.
